What you should know

Our programs involve medically supervised treatments. Understanding how they work helps you make an informed decision about your care.

Safety & Screening

Every patient completes a standardized intake and contraindication screening before any treatment is initiated. This protects your safety and ensures clinical appropriateness before your first treatment begins.

  • Contraindication screening completed at intake — before treatment
  • GLP-1 screening includes thyroid history, GI history, and more
  • Hormone therapy screening includes PSA, hematocrit, sleep apnea, and cardiac history
  • Signed informed consent required for all treatment pathways
  • Adverse event protocol in place for between-visit concerns

Lab Work & Monitoring

All programs include lab ordering through Clinical Labs. Basic labs are often billed through insurance when applicable. A virtual lab review with Kahi is included within 72 hours of receiving results.

  • Baseline labs completed before treatment begins
  • Ongoing monitoring included in Tier 2 and Tier 3
  • Advanced longevity markers available in Tier 3
  • Labs ordered separately — out-of-pocket options available if insurance doesn't apply

Testosterone Therapy

Testosterone replacement therapy (TRT) and enclomiphene are evaluated individually based on symptoms, bloodwork, and health history. Hormone therapy is never initiated without proper clinical screening.

  • Full lab panel completed before any hormone therapy begins
  • Individualized dosing and monitoring throughout treatment
  • Ongoing lab monitoring included for active members
  • Important: TRT suppresses spermatogenesis — fertility considerations are discussed and documented prior to enrollment
  • Not appropriate for all patients; eligibility determined at consultation

Our Approach to GLP-1 Medications

GLP-1 medications (such as semaglutide and tirzepatide) are tools — not the entire plan. When clinically appropriate, they can be a powerful part of a broader strategy focused on metabolic health, behavior change, and long-term sustainability.

  • Prescribed only when clinically appropriate after evaluation
  • Used thoughtfully at the lowest effective dose
  • Combined with nutrition coaching and lifestyle support
  • Not appropriate for every patient — candidacy is assessed at consultation
  • Long-term success comes from combining medical therapy with real behavior change

At 6 Months

  • Establishment of a tolerable and effective medication dose (if prescribed)
  • Measurable changes in body composition (weight, waist circumference, or body fat %)
  • Improved consistency in sleep, activity, and daily routine metrics
  • No major adverse effects requiring discontinuation of therapy
At 12 Months

Measurable change

  • Clinically meaningful improvement in metabolic markers (e.g., A1C, fasting glucose, lipids when applicable)
  • Sustained adherence to treatment plan and follow-up schedule
  • Continued improvement or maintenance of body composition changes
  • Reduction in variability of weight and metabolic markers (greater stability)
  • Established routines across nutrition, activity, sleep, and stress management
  • Appropriate titration, maintenance, or de-escalation of medication based on response
At 24 Months

Long-term stability

  • Sustained weight and body composition outcomes without reliance on short-term interventions
  • Long-term maintenance of improved metabolic markers
  • Demonstrated ability to self-manage nutrition, activity, and health behaviors
  • Reduced dependence on pharmacologic intervention where clinically appropriate
  • Low variability in health metrics indicating stability (weight, labs, habits)
  • Ongoing engagement in preventive and maintenance-focused care
Candidacy

Is this program right for you?

Not every treatment is right for every patient. Candidacy is always determined through a proper consultation and lab evaluation — never assumed in advance.

Important screening considerations

  • GLP-1 medications are not appropriate for patients with a personal or family history of medullary thyroid carcinoma or MEN 2 syndrome
  • History of pancreatitis or serious GI motility disorders may affect eligibility for GLP-1 therapy
  • Testosterone therapy is not appropriate for patients with known or suspected prostate cancer, active thromboembolic disease, or decompensated heart failure
  • Patients desiring to preserve fertility should discuss TRT alternatives (e.g., enclomiphene) before enrollment
  • Pregnancy or active breastfeeding is a contraindication to all hormone and GLP-1 therapies offered in this program
